U-PGx Clinician Questionnaire

This is an anonymous questionnaire to evaluate the views and knowledge of current drug prescribers on pharmacogenomics and its role in individualizing patient treatment.

The results of this questionnaire will support the development of a short e-Learning Tool on pharmacogenomics for clinicians and pharmacists.
